Survey Tool
This application was developed for the healthcare industry.
Care of the aged people is gaining more importance as we move
along the 21st century. Certain set of question are asked to the
aged people so as to arrange aids for them. All the set of
questions are dynamically created and are exported to PDF
format. Each question is uniquely addressed to understand the
lifestyle, Interest, hobbies, likes and dislikes of elderly
people. Once the data has been captured, it is stored in a
database and sent via email to the concerned authority to
arrange for the appropriate aid.
